If you consider yourself a part of the GFCC family, we want to help you to get connected in a deeper way. The pages listed to your left are for each respective member of your family to complete - and as you do so, you're helping us to get to know you, get in touch with you, and know how to get you involved better than ever!

A Quick Look at the purpose of each form you'll see...
*Family Connection Form - provides us with general information for your whole Family.
*Head of Household Form - provides us with personal information for all Heads of Household.
*Spouse Form - provides us with personal information for the Spouse of the Head of Household.
*Child Over 12 Form - provides us with personal information for each dependent in the home age 12 or older.  One form submitted for EACH child age 12 or older!

*SINGLES - please complete BOTH a Family form AND a Head of Household form.  Thank You!
